D.S.A. Enterprises offers numerous products and services for the visually impaired as well as for the sighted. For this reason, our web site has been designed with our visually impaired customers in mind, and we have designed our order form with special captions for customers using speech. Below is a list of the ten easy steps to ordering one of our products! After you have opened our Order Form page you would follow these steps.the
Enter the number of products you would like.
In order to make your product easier to find in our inventory, and expedite the processing of your order, each item we sell has a product number. These product numbers are the numbers you must use to order the items you want. The product number is located beside the product you would like to purchase.
Fill in the "Product #" box on the order form EXACTLY how it appears beside the product name.
You also need to fill in the product name just in case there is a mistake with the product number. If your purchase requires any personalization, you can also include it in this field. Any color or print format ( print or Braille ) information must also be included in this field. If you run out of room, or if the product name is too long, only type as much as will fit. Make sure that the personalization, color, and print format is visible.
Enter the price of a single, individual item in this field.
Calculate the total price of the quantity of the item you would like to order. We will use this price column to find the sum of the grand total of your purchase.
Add all of your total prices to find your sub‑total. and then Enter this number in the sub‑total field.
Examine the Shipping Chart located below the order form to find the amount for shipping, you must add to your sub total. Enter this amount in the shipping field.
If you live in Pennsylvania, you must add a 6% sales tax to your order. If not, leave this field blank.
Calculate the final cost of your order by adding up the sub total, shipping cost, and applicable sales tax. Enter this amount in the grand total field.
Fill in all the customer information that we will need to process your order. Although your phone and E-Mail information is optional, we ask that you please include it. We can use it to contact you quickly in case there are any unforseen problems in processing your order. The other information is required for your order to be processed. To respect your privacy, all customer information is strictly for the use of D.S.A. Enterprises of PA, Inc. NO information about you will be lent, shared, or sold to any other organization, business, or mailing list.
Finally, after completing your order, mail it along with a check or money-order for the full price of your purchase to:
